Frequently asked questions about Claims
We make filing claims quick and easy. In most cases, simply download and complete your claim form, then mail it to us. For Health policies and ongoing long-term care claims, you can upload your claim online by logging into My Account or with the PM My Account app (iOS)(Android). For new long-term care, home health care or accidental death insurance policy claims, please call us between the hours of 8 a.m. and 5 p.m. Central Time, Monday through Friday, at 1-800-228-9100. Make sure you have your insurance policy number and date of birth when you call.
If you have a dental claim or benefit questions, please call Ameritas at 1-877-667-6187 between 7 a.m. and 12 a.m. Central Time, Monday through Thursday or 7 a.m. to 6:30 p.m. on Fridays. Please have your policy/certificate number (from your ID card) and date of birth when you call.
Most dentists offer the service of filing dental claims for their patients. If your dentist doesn’t file the claim, you may submit it on your own. You have two options:
Option 1: Download and complete a dental claim form. After you complete the top portion, have your dentist fill out the bottom. Then, send the form to Ameritas.
Option 2: Send Ameritas the original copy of the itemized dental bill (the one with the dental procedure codes). Be sure to send a cover letter with your contact information and insurance policy/certificate number.
