There’s no need to worry if you’ve misplaced or accidentally damaged your insurance policy, billing statement or health identification card. We’ll be happy to send you replacements. Just log in to My Account and fill out the brief online form or, if you prefer, simply download the form and mail it back to us. Then, we’ll send you the documentation you need, normally free of charge.

Changing a beneficiary is an important decision, so we’ll take great care to honor your request. Please complete and mail the Change of Beneficiary form to begin the process.

Your insurance policy number appears on your coverage documents and most correspondence you receive from us. You may also find it on your ID card. Or, simply contact a customer service representative at 1-800-228-9100. For annuities, please call 1-800-720-2891. You’ll be asked to provide your full name, address and date of birth when you call.
