How do I file a claim?

We make filing claims quick and easy. In most cases simply download and complete your claim form, then mail it to us. For long-term care, home health care or accidental death insurance policy claims, please call us between the hours of 8 a.m. and 5 p.m. Central Time, Monday through Friday, at 1-800-228-9100. Make sure you have your insurance policy number and date of birth when you call.

If you have dental claim or benefit questions, please call Ameritas at 1-877-667-6187 between 7:00 am and 12:00 am (Central Time) Monday through Thursday or 7:00 am to 6:30 pm on Fridays. Please have your policy/certificate number (from your ID card) and date of birth when you call.

Most dentists offer the service of filing dental claims for their patients. If your dentist doesn't file the claim, you may submit it on your own. You have two options:

Option 1: Download and complete a dental claim form. After you complete the top portion, have your dentist fill out the bottom. Then, send the form to Ameritas.

Option 2: Send Ameritas the original copy of the itemized dental bill (the one with the dental procedure codes). Be sure to send a cover letter with your contact information and insurance policy/certificate number.

How do I check the status of my claim?

Simply call our automated 24-hour line at 1-800-228-9100 to check the status of your claim. To check the status of your dental claim, please call 1-877-667-6187. Make sure you have your insurance policy number and date of birth when you call.

Is direct deposit available for benefit checks?

We understand that getting to the bank is not always convenient. However, for your security, we don't offer direct deposit for benefit drafts. Sorry!
