How do I file a claim?

We make filing claims quick and easy. In most cases simply download and complete your claim form, then mail it to us. For long-term care, home health care or accidental death insurance policy claims, please call us between the hours of 8 a.m. and 5 p.m. Central Time, Monday through Friday, at 1-800-228-9100. Make sure you have your insurance policy number and date of birth when you call.

Is direct deposit available for benefit checks?

We understand that getting to the bank is not always convenient. However, for your security, we don't offer direct deposit for benefit drafts. Sorry!
